Thank you SO much for allowing us to use your child in our marketing! We promise to always use the photos in a respectful manner to promote our business & nothing else!

This Model Release Agreement (the “Agreement”) is made and entered into as of the date signed below by the undersigned parent or legal guardian (the "Guardian") of the minor child (the "Model") named below, in favor of [Photographer’s Name] (the "Photographer").

1. Consent for Use: I, the undersigned Guardian, hereby grant permission to the Photographer to photograph, record, and use the likeness of my child, [Child’s Name], in any form of media, including but not limited to digital and print advertising, social media, website galleries, promotional materials, and other marketing efforts.

2. Rights and Ownership: I understand and agree that the Photographer owns the copyright to all images taken and may use them for promotional and commercial purposes. I acknowledge that I will not receive any compensation or royalties for the use of these images.

3. Waiver of Claims: I waive any rights to inspect or approve the finished product in which my child’s likeness appears. I also release and discharge the Photographer and any affiliates from any claims, demands, or liabilities arising out of the use of the photographs.

4. Confidentiality & Privacy: I understand that my child’s full name and personal information will not be disclosed without my explicit consent.

By filling out the form below, I confirm that I am the legal guardian of the minor named herein and have the authority to grant this release.
